Service Pledge

  • Provide potable, sufficient and affordable water to the consuming public;
  • Accommodate the concessionaires as soon as they enter the premises of the District;
  • Respond promptly to requests and complaints about our services and take corrective measures soonest or within the day;
  • Adhere to the norms and conduct as public officials to be courteous at all times in dealing with the public and our colleagues in service;
  • Promote inter-personal relationships among employees for effective and efficient delivery of service.

MCWD Frontline Services

REQUIREMENTS:

  1. Accomplished:
  2. Proof of Ownership (any of the following whichever is applicable):
    • Transfer Certificate Title
    • Latest Tax Declaration
    • Deed of Sale
    • Award Certificate (Housing Units)
    • Conditional Sale or Certification from developer in-case award is not available yet
    • Lease Contract
  3. Proof of Residence:
    • Certificate of Occupancy from the Barangay Government as proof that the applicant is residing thereat.
  4. Others:
    • Authorization from owner, in case property where the connection will be installed is not owned by the applicant.
    • Affidavit of Heirs, in case the property is not subdivided yet to the beneficiaries, Mother Title should also be attached.
    • Contractor's Permit and Authority to Commence (for construction)
    • Business Permit (in lien of the certificate of occupancy, if the connection is classified under commercial category)
    • Affidavit of Undertaking (for connections within the bank allowances and other areas considered as government properties)
  5. 1 x 1 ID pictures (2 pcs)


PROCEDURE:

  1. APPROACH the Customer Services desk, located at the front desk upon entrance to the MCWD Building and ask for requirements on applying for a New Water Connection
  2. Submit your duly accomplished requirements to the Customer Service Assistant assigned at the Customer Services desk
  3. Follow further instructions given by the Customer Services Assistant

REQUIREMENTS:

  1. Accomplished:
  2. Payment of Billing Account and Reopening Fee
  3. Others:
    • Special Power of Attorney (SPA), in case the registered concessionaire is not available to sign and transact.
    • Death Certificate, in case the registered concessionaire is already deceased.
    • Birth Certificate together with the Affidavit of Heir for the applicant requesting the reopening of closed connection.
    • Affidavit as Caretaker in lieu of the SPA
    • Waiver of Rights (in favor of the concessionaire who assumes the connection)


PROCEDURE:

  1. APPROACH the Customer Services desk, located at the front desk upon entrance to the MCWD Building and ask for requirements on applying for a New Water Connection
  2. Submit your duly accomplished requirements to the Customer Service Assistant assigned at the Customer Services desk
  3. Follow further instructions given by the Customer Services Assistant
REQUIREMENTS:

  1. Accomplished:
  2. Waiver of Rights from previous registered concessionaire.
  3. Death Certificate together with Affidavit of Heirs
  4. Marriage Contract, in case of change name to wife or to husband.


PROCEDURE:

  1. APPROACH the Customer Services desk, located at the front desk upon entrance to the MCWD Building and ask for requirements on applying for a New Water Connection
  2. Submit your duly accomplished requirements to the Customer Service Assistant assigned at the Customer Services desk
  3. Follow further instructions given by the Customer Services Assistant
REQUIREMENTS:

  1. Affidavit that any form of business engaged is no longer existent (downward reclass)
  2. Certification from barangay to confirm the above statement


PROCEDURE:

  1. APPROACH the Customer Services desk, located at the front desk upon entrance to the MCWD Building and ask for requirements on applying for a New Water Connection
  2. Submit your duly accomplished requirements to the Customer Service Assistant assigned at the Customer Services desk
  3. Follow further instructions given by the Customer Services Assistant
REQUIREMENTS:

  1. Accomplished Application for Senior Citizen's Discount Card Form
  2. Birth Certificate
  3. Marriage Contract, in case applicant is married.
  4. Certificate of Residency
  5. 1 x 1 picture (2 pcs)
  6. Photocopy of Senior ID (Local/National)

  2. Submit your duly accomplished requirements to the Customer Service Assistant assigned at the Customer Services desk
